This form is for members of the public who seek to view or obtain a copy of public records held by the District as spelled out by the Illinois Freedom of Information Act. The form is not required, and requesters can submit a request in any form, including verbal in-person requests, email, fax, or hand-written. Requests typically go directly to the FOIA Officer, as described on the website. If a request for public records comes to an employee other than the FOIA Officer, please forward to the FOIA Officer as soon as possible at FOIA@district158.org or the Superintendent’s Office.
