The Heineman Middle School Office will be closed June 29 – July 15, 2019. There may be additional days when administration is present, but not in the Heineman office.
Parents/guardians seeking to submit medical forms may do so via fax at (847) 659-4320, and they will be printed when the secretarial staff returns on Wednesday, July 31. Medical records also may be submitted via email by visiting www.district158.org/parents/registration/medical-records/ and clicking “Heineman Middle School.”
