Athletic Clearance-to-Participate Procedure

A student-athlete needs to do the following before participating in any athletic activity:

• Submit a current physical form to the HHS Athletic Office
• Submit a completed Sports Participation Information form to the HHS Athletic Office (once per school year)
• Pay the participation fee, of $75 per sport, in the athletic office

The Sports Participation Information form and physical forms are available in the HHS Athletic Office and on the District 158 Website.

The physical and information forms must be turned in before being allowed to practice*.

The participation fee is due before the first contest.

The student-athlete will be given a “clearance card” to begin practice* when he or she submits the required paperwork. The student-athlete needs to present the “clearance card” to his or her coach.

The student-athlete will be given a “clearance card” to compete when he or she pays the $75 participation fee. The student-athlete needs to present the “clearance card” to his or her coach.

* In some sports, the first few days of practice are a tryout period.
