 RECENT PHYSICAL
SPORTS PARTICIPATION INFORMATION SHEET
ON FILE IN THE HHS ATHLETIC OFFICE BEFORE BEGINNING PRACTICE

NEW FOR 2008-09
No Student-Athlete may participate in
IHSA state series competition
(Regional, Sectional, etc.)
unless the student and parent/guardian
consent to random testing
