			<content:encoded><![CDATA[<p></p><h3><span style="color: #000000;">Numerous outbreaks of pertussis (whooping cough) have occurred recently among school children in Illinois.  Pertussis is easily transmitted through coughing and sneezing and may cause an illness that persists for weeks to months. Pertussis does not typically cause severe illness in healthy students, but can cause prolonged absences from school and extracurricular activities. In addition, pertussis can be transmitted from healthy students to infants and individuals with chronic illnesses, for whom pertussis can be life-threatening.  </span><span style="color: #000000;"> </span></h3>
<h3><span style="color: #000000;">Protection against pertussis begins to wear off during grade school. This leaves pre-teens, teenagers and adults at risk for this illness. To address the increase in pertussis cases among older students, a booster vaccination (called Tdap) is recommended for all students in grades 6 through 12.</span><span style="color: #000000;"> </span></h3>
<h3><span style="color: #000000;">This fall, students entering grades 6 and 9 will be required to provide proof of Tdap vaccination along with the school physical forms that are also required for these grades.    </span><span style="color: #000000;"> </span></h3>
<h3><span style="color: #000000;">Students entering grades 6 or 9 without one of the following will be subject to exclusion:</span><span style="color: #000000;"> </span></h3>
<h3><span style="color: #000000;">1. Proof of Tdap vaccination</span></h3>
<h3><span style="color: #000000;">2. An approved medical or religious exemption on file with the school, </span></h3>
<h3><span style="color: #000000;">3. An appointment to receive the Tdap shot during the school year.  </span></h3>

<h3><span style="color: #000000;">Many providers, local pharmacies and most local health departments provide Tdap vaccinations. Many providers participate in the Vaccines for Children (VFC) program, which provides vaccines at no cost to doctors who serve Medicaid-eligible children younger than 19 years of age. If you need assistance, check with your local health department for resources for getting Tdap vaccination.  </span><span style="color: #000000;"> </span></h3>
<h3><span style="color: #000000;">Check with your doctor if you are not sure if your child has received Tdap, and if not, be sure to get your child vaccinated.</span></h3>

<h3><span style="text-decoration: underline;"><span style="color: #800000;">Last Day of School</span></span></h3>
<ul>

<h3><span style="color: #000000;">The official last day of school for the 2011-2012 school year will be Friday, June 1.</span></h3>
</li>
</ul>
<h3><span style="text-decoration: underline;"><span style="color: #800000;">8th Grade Promotion Ceremonies</span></span></h3>
<ul>
<li>
<h3><span style="color: #000000;">Promotion ceremonies for 8th grade students will be held on Thursday,<br />
May 31. Specific information for each middle school ceremony will be posted at a later day. Please check back.</span></h3>
</li>
</ul>
<h3><span style="text-decoration: underline;"><span style="color: #800000;">Graduating Seniors</span></span></h3>
<ul>
<li>
<h3><span style="color: #000000;">The last day of attendance for graduating seniors will be Friday, May 25. All textbooks need to be returned to the bookroom no later than 3:00 p.m., unless the student is taking a final exam.</span></h3>
</li>
<li>
<h3><span style="color: #000000;">Graduation practice will be held on Friday, June 1 at 1:00 p.m. for all students who plan on participating in the graduation ceremony.</span></h3>
</li>
<li>
<h3><span style="color: #000000;">Graduation ceremonies will take place on Saturday, June 2 at the Sears Centre beginning at 2:00 p.m.</span></h3>
</li>
</ul>
<h3><span style="text-decoration: underline;"><span style="color: #800000;">Food Services Department</span></span></h3>
<ul>
<li>
<h3><span style="color: #000000;">Parents are asked to please check your child&#8217;s account balances online and report any discrepancies to the school cashier by May 25.</span></h3>
</li>
<li>
<h3><span style="color: #000000;">No charges will be extended after May 18. This means that there needs to be money in the student&#8217;s account &#8211; it does not mean that you cannot use MyLunchMoney.com.</span></h3>
</li>
<li>
<h3><span style="color: #000000;">Funds in your child&#8217;s account at the end of the year will remain in the account and will be available next school year regardless of which District 158 school they will attend. </span></h3>
</li>
<li>
<h3><span style="color: #000000;">Lunch Menu &#8211; During the month of May, we ask parents to check the lunch menus each week as menus may be revised in an effort to eliminate waste.</span></h3>
</li>
</ul>

<h3><span style="color: #000000;">Lunch for Last Day of School &#8211; Due to the number of end of year activities scheduled for the last day of school, the District Food Services Department has decided to serve a cold lunch/sack lunch on Friday, June 1.</span></h3>

<h3><span style="color: #000000;"><span style="color: #ff0000;">If you are moving <span style="text-decoration: underline;">out of District 158</span> and have a MyLunchMoney.com auto replenish set for your child&#8217;s account, you must turn it off. <span style="color: #000000;">If you have any questions, please contact District 158 Food Services at (847) 659-6155.</span></span></span></h3>

<h3><span style="text-decoration: underline;"><span style="color: #800000;">Copies of Student Records/Forms</span></span></h3>

<h3><span style="color: #000000;">Many summer camps and athletic programs require a copy of the student&#8217;s birth certificate and/or a copy of the student&#8217;s most recent physical form. Parents can obtain photocopies of these items <span style="text-decoration: underline;">from the school office</span> for a fee of $.05 per page. Photo identification from the parent/guardian is <span style="text-decoration: underline;">required</span>. It is recommended that you call the school office to ensure an office staff member will be available to assist you with your request. Please note these items are NOT available through the District 158 Registration Department.</span></h3>

<h3><span style="text-decoration: underline;"><span style="color: #800000;">Immunization and Physical Forms for 2012-13</span></span></h3>

<h3><span style="color: #000000;">Physical forms and immunization records for the 2012-13 school year must be dropped off, mailed, or faxed <span style="text-decoration: underline;">to the school your child will attend in the fall</span>. Parents are encouraged to keep a copy for their records.</span></h3>

<h3><span style="color: #000000;">Students entering Kindergarten will not receive their teacher assignments until their physical is submitted.</span></h3>

<h3><span style="color: #000000;">Students who are entering grade 6 or grade 9 will not be permitted to pick up their schedule at &#8220;Material Pick-Up Day&#8221; without a physical submitted.</span></h3>
