Illinois State Law requires children entering Kindergarten, grade 6 and grade 9 have a new complete physical examination performed with a copy of the exam on file with the Health Office before school begins or your student will not be allowed to begin school.
Mid-year transfer students will have 30 days from the enrollment date to have the following information submitted. All students who are new to Illinois schools also need a complete vision exam.
Students must have a current physical exam to participate in interscholastic sports. Students may NOT try out for a sport without a physical exam. The physical must have been performed within 13 months prior to the start of participation, including tryouts and/or conditioning.
All sports physicals submitted and dated after September 1, 2016 will remain current for 395 days.
