Illinois State Law requires children entering Kindergarten, grade 6 and grade 9 have a new complete physical examination performed with a copy of the exam on file with the Health Office before school begins or your student will not be allowed to begin school.
Mid-year transfer students will have 30 days from the enrollment date to have the following information submitted. All students who are new to Illinois schools also need a complete vision exam.
Students must have a current physical exam to participate in interscholastic sports. Students may NOT try out for a sport without a physical exam. The physical must have been performed within one year of the start of the specific sport.
All sports physicals submitted and dated after September 1, 2014 will remain current for 395 days.
